Refund & Purchase Policy

Payment for all classes must be received prior to the scheduled class date to ensure your space. Private bookings are only booked in and confirmed with advance payment. Your session may not go ahead if we have not received payment, and it will not be booked in if it is your first visit.
Our Studio has a minimum age of 21+
All purchases are final and non refundable. Purchases made in error will be subject to a 6% transaction fee should a refund be required.
Single and Class Packs are active from date of purchase. Monthly Memberships are on a 3 month rolling subscription until cancelled. There is a standard 14 day cooling off period after each new Subscription Renewal. 1 Months notice in writing is required to cancel the contract.
Forms Cancellation policy is 24 hours. No shows and late cancellations are charged at the rate of the booking fee. Cancellations need to be made via your Momence account. Booking or cancellation requests made via email will not be accommodated.
In extenuating circumstances if Form has to cancel a session, your class will be credited back to your account as a class credit.
Class & Block Expiry dates are from purchase date and are not eligible to be extended.
Gift Cards have a 6 Month Expiry from Purchase.
Gift Cards are non Transferable and purchased via Momence.

Form runs a strict 24 hour cancellation policy on all group classes. All Classes cancelled outside of this time are charged at full class price.
No Shows are charged as if you attended and a class is deducted from your block/membership accordingly.
Private bookings require payment direct to the Studio and have a 48 hour cancellation policy. Cancellations or re-schedules made after this time are charged at full price.
All cancellations must be made through the Momence App.
Cancellations made via email or through Social Media will not be accommodated.
